ID RequirementsGovernment-issued photo ID required. Name must match NABP e-Profile exactly. Two forms of ID may be required at some testing sites.
RenewalRequired -- Pharmacist licensure renewal requirements vary by state. ExCPT (NHA) requires renewal every 2 years with continuing education. NAPLEX score itself does not expire for most states (check individual state board rules).
